This consent describes telehealth services that may be provided by participating licensed healthcare providers and their medical practices through or in connection with MOVA. It supplements, and does not replace, consent for a particular treatment, medication, procedure, or disclosure of health information.
2. What telehealth is
Telehealth uses electronic communications to exchange medical information and support evaluation, consultation, follow-up, education, prescribing when appropriate, and care coordination. Interactions may use questionnaires, secure messaging, phone, video, images, records, or connected tools.
3. Potential benefits
Potential benefits include more convenient access, reduced travel, faster exchange of information, and easier follow-up. These benefits are not guaranteed and depend on your condition, location, technology, and provider availability.
4. Risks and limitations
Telehealth risks and limitations may include:
- incomplete information without an in-person examination;
- delays, disconnections, equipment failure, or poor image quality;
- unauthorized access or privacy risks despite safeguards;
- the need for in-person examination, testing, or emergency care;
- limits on prescribing or treatment based on location or law.
5. Alternatives and choice
You may ask questions, decline telehealth, request an available alternative, or seek in-person care. Declining telehealth may mean the requested MOVA pathway cannot be provided, but it does not prevent you from seeking care elsewhere.
6. Privacy, security, and recording
Participants should use a private location, secure device, and trusted network when possible. Tell the provider if another person is present. A session will not be recorded unless you are informed and separately consent where required. Privacy practices are described in the Privacy Policy and any applicable provider Notice of Privacy Practices.
7. Your identity, location, and responsibilities
You agree to provide accurate identity, contact, location, and health information; disclose relevant medications and allergies; follow safety instructions; and tell the provider if your symptoms change. You may be required to confirm your physical location during each encounter. Telehealth is available only in jurisdictions where the relevant clinicians, pharmacies, and services are legally available.
8. Emergency and urgent care
Telehealth through MOVA is not for emergencies. Call 911 or your local emergency number for severe symptoms, immediate danger, or a potentially life-threatening condition. Do not wait for an online message or appointment.
9. Independent clinical judgment
The provider independently determines whether telehealth is adequate and whether any diagnosis, treatment, prescription, refill, test, or referral is appropriate. Completing an intake or paying a fee does not guarantee treatment, a specific medication, or a particular result.
10. Medical records and care coordination
Information from a telehealth interaction may become part of the medical record and may be shared with clinicians, pharmacies, laboratories, or other care partners as permitted by law and applicable privacy notices. You may be asked to authorize additional disclosures when required.
11. Fees and insurance
Product, clinical evaluation, laboratory, pharmacy, and shipping charges may be separate unless checkout expressly presents an inclusive price. Insurance coverage is not guaranteed. Review the price, cancellation, and refund terms shown before payment and in the Terms of Use.
12. State-specific requirements
Telehealth consent and prescribing rules vary by jurisdiction. Before clinical care begins, the provider will confirm your physical location and provide or obtain any additional notice, verbal confirmation, or written consent required there. The website intake does not by itself determine or satisfy every location-specific requirement.
13. Withdrawal of consent
You may withdraw telehealth consent before a future service by contacting MOVA or the treating provider. Withdrawal does not affect care already provided, records already created, or uses of information already permitted by law. It may prevent completion of an online care pathway.
14. Acknowledgment
By checking the separate telehealth consent box in the intake flow, you confirm that you have read this document, understand the benefits, risks, alternatives, privacy considerations, and emergency limits, and voluntarily consent to telehealth. A provider may confirm your consent again at the start of care.
